Carriglea Cáirde Services is committed to providing effective, safe and person centred supports to all those who use our services. The Service is regulated by the Health Information and Quality Authority (HIQA) under the provisions of the Health Act 2007.

We aim to meet the highest possible standards as set out by HIQA the in the National Standards for Residential Services for Adults with Disabilities.

Through our person-centred approach to service provision, we strive to respect people’s rights at all times and to support each person to achieve his/her full potential.

Carriglea Cáirde Services operates a Quality Management System which encompasses a comprehensive range of policies and procedures covering all aspects of governance and service delivery, health and safety and human resources management.

 

Person Centred Planning

All service users are given the opportunity to have their own person centred plan. The person centred plan sets out the individual’s aims and objectives and identifies the supports required to achieve these objectives. The person centred planning process involves:

  • Assessment:- Identifying the individual’s requirements by completing an information gathering questionnaire with the service user in conjunction with staff and the person’s chosen representative and wider circle of support.
  • Planning:- Setting out a comprehensive plan to achieve the goals and objectives indentified in the assessment. Identifying the supports required to maximise the person potential to have a good quality of life.
  • Implementing:- Putting in place the services to meet the care plans and action plans required to meet the person’s needs and to achieve their goals.
  • Evaluation:- Regularly assessing and documenting the outcomes of the care plans and action plans put in place and making any required changes to the plan in accordance with the service user’s wishes and changing needs

Each service user has a key worker of their choice who takes a special interest in supporting the person to achieve the goals and objectives of his/her person centred plan and to live a life of his/her choosing.
